This form, upon completion to a reasonable degree (ie not mostly empty) is worth 400SP.  You may complete it for SP only once per character. 

Characters carried over from Chronicle One are encouraged to participate. (Many of the backstories were lost when the previous website went down.)

You may resubmit the form to fine tune your back story (so long as you include everything again so we can remove the original entry), but you will only get the SP once.
